The Cost of Services*

Primary Care $

Primary Care visit is covered at 100%. Coinsurance and deductible may be applicable if testing ordered.

Telehealth $

Primary Care telehealth is covered at 100%. Coinsurance and deductible may be applicable if testing ordered.

Same-Day Care $

Same-Day Care visit is covered at 100%. Coinsurance and deductible may be applicable if testing ordered.

Traditional Urgent Care $

Traditional Urgent Care visit is covered at 80%. Coinsurance and deductible may be applicable if testing ordered.

Advanced Urgent Care $$

Visit is billed at a specialist rate, not at an urgent care rate. Plan pays 80%, deductible may apply. Any other services such as lab and imaging will hit their respective fee schedules.

Orthopedic Urgent Care $$

Visit is billed at a specialist rate, not at an urgent care rate. Plan pays 80%, deductible may apply. Any other services such as lab and imaging will hit their respective fee schedules.

Emergency Department $$$$

All patients will be treated regardless of their ability to pay. National average estimated at $1,500. Coinsurance and deductible applicable.
