What is Prime Care Connect?
Prime Care Connect is medical coverage that is designed for those with limited household income and intended to reduce some of the financial reasons that may prevent members from getting appropriate and timely health care. The Ohio State University Health Plan provides support to help determine those who are qualified to participate in this plan.
Eligibility Requirements
Before filling out your Prime Care Connect application, please make sure that these four statements are true:
1. I am not an employee of an affiliate group (Central Ohio Technical College, OSU Physicians) or a graduate associate.
2. I am a full-time university employee (75-100% FTE) in a regular, term or seasonal appointment.
3. I am applying during Open Enrollment or because of a qualifying event.
4. My adjusted gross household income1 does not exceed the maximum amount in the table below for the number of people in my household2.

1. Income is determined by previous year's adjusted gross income.
2. Your household includes all persons residing in your home, even those who are not enrolled. Note: the adjusted gross income of every person residing in your home will be added to your total household income, even if that person is not an eligible dependent. Your household also includes any dependent who is going to be enrolled and lives outside of your home.
How Do I Apply?
To apply for Prime Care Connect, you can:
- Login to Link to Health and apply online.
- Request an application be sent to you from the OSU Health Plan at OSUHealthPlanCS@osumc.edu, 614-292-4700, or 800-678-6269.
- Download and fill out the Prime Care Connect application within the PDF or print and fill out. You can then:
• Email the application and required documents to OSUHealthPlanCS@osumc.edu
• Fax the application and required documents to 614-292-2667
• Mail the application and required documents to: OSU Health Plan, 700 Ackerman Road, Suite 1007, Columbus, Ohio 43202
If your application was approved and made due to a qualifying event, Prime Care Connect coverage will be effective on the event date. If approved and the application was made during Open Enrollment, coverage will be effective at the beginning of the next plan year on January 1.
A letter will be sent from OSU Health Plan advising you of the final eligibility determination. You are encouraged to enroll in one of the university’s other medical plans and if your application is approved, your coverage will be automatically changed to Prime Care Connect. If not, your coverage will remain your chosen plan.
*A decrease in household income during the plan year is not an event that allows enrollment during the plan year; household income is based on the prior year’s tax return.

The employee contribution amount for Prime Care Connect is the same as for Prime Care Advantage, but the member and their dependents will have lower out-of-pocket cost due to the plan’s lower copayments, coinsurance and out-of-pocket maximum amounts.

1. A PCP is a generalist physician designated as a family medicine, general internal medicine, geriatric medicine or general pediatrics provider. Primary care services can also be provided by a primary care nurse practitioner or physician assistant who practices with a PCP. This benefit also applies to Employee Health and Urgent Care and Express Care visits.
Health Care Doesn't Have to Be Expensive.

You will be asked to complete and sign a brief application, provide proof of household income along with any other necessary documentation, and return the completed paperwork to OSU Health Plan. All applications are treated as confidential. Eligibility approvals will be sent to the Office of Human Resources for processing plan enrollment; however, no information contained in the application is made available to your employer.
